When making a referral please ensure that you include as much detail as possible for the reason(s) for the referral, including any local support you are providing.
If you are referring for exam failure, please include the number of exam attempts, together with information on how the Postgraduate Doctor or Dentist in Training is progressing with other areas of the curricula.
